A critical examination of translational medicine, when private risk is transferred to the public sector and university research teams become tech startups for global investors. A global shift has secretly transformed science and medicine. Starting in 2003, biomedical research in the West has been reshaped by the emergence of translational science and medicine--the idea that the aim of research is to translate findings as quickly as possible into medical products. In The Market in Mind , Mark Dennis Robinson charts this shift, arguing that the new research paradigm has turned university research teams into small biotechnology startups and their industry partners into early-stage investment firms. There is also a larger, surprising consequence from this shift: according to Robinson, translational science and medicine enable biopharmaceutical firms, as part of a broader financial strategy, to outsource the riskiest parts of research to nonprofit universities. Robinson examines the implications of this new configuration. What happens, for example, when universities absorb unknown levels of risk Robinson argues that in the years since the global financial crisis translational science and medicine has brought about "the financialization of health." Robinson explores such topics as shareholder anxiety and industry retreat from Alzheimer's and depression research; how laboratory research is understood as health innovation even when there is no product; the emergence of investor networking events as crucial for viewing science in a market context; and the place of patients in research decisions. Although translational medicine justifies itself by the goal of relieving patients' suffering, Robinson finds patients' voices largely marginalized in translational neuroscience.

La raison en est simple : comme, en France, 92 % du coût des soins médicaux ne sont pas supportés par le patient au moment de ses soins, puisqu ils sont presque toujours gratuits, la demande du patient est potentiellement infinie, elle doit donc être régulée. Si elle ne peut pas l être par des mécanismes de marché, l État est contraint de contrôler de façon autoritaire le montant et/ou les quantités de ressources financières, de biens ou de services affectés aux soins médicaux. La vérité est donc qu il rationne, même si l euphémisme utilisé est de dire qu il « régule ». Tout ou presque est rationné : le nombre de médecins (numerus clausus), les lits hospitaliers, les pharmacies, les équipements lourds, les actes médicaux (nomenclatures), les médicaments remboursés par l assurance maladie. Il n y a de prix que pour les médecins du secteur 2 et pour quelques services (chambres particulières) ou biens médicaux ; la règle est le tarif basé sur des nomenclatures et les tarifs ne sont pas des prix. Si, dans le domaine des soins médicaux, les mécanismes de marché sont inégalitaires et inflationnistes, si l on est éthiquement contraint de recourir à des mécanismes de rationnement, comment les penser et les gérer ? Peut-on dans certaines circonstances, grâce au marché, ne plus sentir peser la chape du rationnement ? Tel est l objet de cet ouvrage qui traite de la question centrale en économie : l accès et la répartition des biens rares."

Abstract

This book presents different patient-oriented perspectives from surgeons, economic evaluation and management researchers, and business companies active in the healthcare sector, striking a balance between the appropriateness/effectiveness of treatment and efficiency/cost. It does not include technical surgical details, but instead provides the necessary knowledge regarding different groups of patients to help economic and management researchers make accurate evaluations. Although partially based on the specific case of abdominal wall surgery in the Italian health system, the book defines a model that can, with the necessary adaptations, be applied in other national contexts. It also analyzes different reimbursement systems and methods of data collection. This approach supports the evolution form evidence-based medicine (EBM) to the future of real-world data (big data analysis). Further, it highlights the critical issue of “silos” reimbursement, which is the pillar of DRG, and proposes methodology to evaluate the direct and indirect benefit and costs of surgery (for example quality of care, costs incurred in cases of surgical complications due to the use of inappropriate, low-cost material or due to surgical procedure. It is a valuable resource for clinicians, surgeons, policymakers and managers in the field.
